It is very important that your current address and/or contact details are held by the Directorate of Honours and Awards to ensure that your medals are delivered to the correct address.
If you have changed address since you submitted your application, please download a copy of the Change of Address and/or Contact Advice form and post it to the Directorate at the address shown on the form.
If you are a currently serving Australian Defence Force member, do not use the Change of Address and/or Contact Advice form as your details that appear on PMKeys will be used for assessment and award delivery purposes. As such, please ensure that any changes to your details are recorded on PMKeyS.
